Please complete this form if you child will require any medications at camp (this includes scheduled and unscheduled medications).

A small bag will be sent home with your child on Wed 2/4/25 for you to supply this medication to us before camp.

Please ensure these medications are in their original packaging with name and dosage clearly labelled.

Medications for camp need to be handed into the school office the day before camp - Friday 4th April.

Please note: We cannot administer any medication, including over the counter medication, without the below information completed and medication provided to the school before the morning of camp.
