This form serves as a continuous permission note for participation in school rugby league or rugby union programs, trials, training and games throughout the year.

Should you allow your child to be part of Waratah’s football teams, coaches will still remind you of upcoming games and other information and a signed permission note will still be required for each event.

Please sign this consent form relating to medical conditions, injury history, experience and preferred playing positions.
