MEDICAL INSURANCE

Parents please note there is no personal injury insurance cover provided by the NSW Department of Education and Training for students in relation to school sporting activities, physical education lessons or any other school activity. Parents and caregivers are advised to access the level and extent of their child’s involvement in the sport program offered by the school, school zone, area and state school sport association when deciding whether additional insurance cover, above that provided by Medicare, is required. Personal accident insurance cover is available through normal retail insurance outlets. The NSW Supplementary Sporting Injuries Benefits Scheme, funded by the NSW Government, covers any injury resulting in the permanent loss of a prescribed faculty or the use of some prescribed part of the body. Please note that ambulance cover for accidents is not provided in states other than NSW.

PUBLICATION PERMISSION

I give permission for the school/department to publish information, images, and/or videos of my child in publicly accessible communications. I understand this consent remains effective until I provide written notice withdrawing it.

EXTERNAL SUPERVISION

I acknowledge that my child may, at times, be under the direct supervision of an external coach during program activities.

PROGRAM FEES

I understand that TSP is an optional program and agree to pay all associated fees on time. I acknowledge that no refunds will be issued for any reason, including withdrawal from the program.

ALTERNATE TRAINING VENUES AND TRANSPORT

I give permission for my child to travel to alternate training venues as required. Transportation may be by their own means, the school bus, or private vehicles of coaches or teachers, as deemed appropriate by the school.

PARTICIPATION IN WATER ACTIVITIES

I give permission for my child to participate in water-based activities, including during periods when council-monitored beaches may be closed. I understand that these activities may occur at any time during the year, including during the Winter Period.

ACKNOWLEDGMENT OF RISK

I acknowledge that surf training, surfing, and other water activities can be hazardous. While all reasonable care will be taken to ensure the safety of participants, I understand and agree that my child participates in these activities at their own risk.

MEDICAL INSURANCE AND LIABILITY

I confirm I have read and understood the information provided regarding medical insurance. I acknowledge that the school and program organisers are not liable for any injuries, accidents, or losses incurred during participation in TSP. I agree to bear full responsibility for any medical or other costs arising from such incidents.

WITHDRAWAL OF CONSENT

I understand that any permissions granted in this form may be withdrawn by providing written notice to the school. However, withdrawal of permission for any single component may affect my child’s ability to participate fully in the program.
