Any changes to the above prescribed medication or dosage must be made in writing by the prescribing doctor and a new form completed by the parent/carer.

A new Patient Medication Summary from the doctor is to be uploaded with this form.

Over the counter medication (not requiring a prescription) must be in the original packaging with the manufacturer's instructions on how to administer the medication.

Prescribed medication must be in the original packaging provided by the pharmacy and include a pharmacy label detailing the student's name & instructions for administration. Blister packs prepared by the Chemist with a pharmacy label with student's name is also acceptable.

All medications are stored and administered from the Health Centre except for on school excursions (other school staff may administer the medication whilst on excursion) or a written permission request from the parent has been approved by the school.

Secure delivery of prescribed medication is important for the safety of your child as well as for the safety of students in the school. Please deliver medications to the Health Centre immediately on arrival to school.

Medication is not to be self-administered or stored outside of the Health Centre (eg. dormitories, bags, etc) unless a written request has been made by the parent/carer and approved by the school.
