An illness/misadventure form must be submitted in the following instances:

A student is unexpectedly and genuinely absent in the lead up to or on the due date of an assessment task
In the case of a genuine absence, the completed and signed form must be submitted within 5 school days (or on return to school).

If you cannot attend school on the day of an assessment task to submit it or complete the task in person because of a valid reason (illness or misadventure) you must do the following:

Have your parent/caregiver contact the school on the day of the task and notify the office of your absence.
Complete this form and attach and relevant documentation.
Submit the task on the first day back at school.
