Exchange of Information – Transport for NSW

Students aged 16 years and over who attend a NSW public school may request that the school apply for a Transport for NSW Student Concession Entitlement Card on their behalf. This card provides official confirmation that the student is enrolled at school and remains eligible for discounted travel using a Concession Opal card.

Privacy and Confidentiality

To process this application, the school is required to provide Transport for NSW with the student's full name, date of birth and current school year through a secure electronic process.

Consent

I consent to St Johns Park High School providing my child's full name, date of birth and current school year to Transport for NSW for the purpose of applying for a Concession Entitlement Card.
