Medication Authority Form

To administer long term medication at school, we require a Medication Authority Form (MAF) signed by the prescribing doctor. A template is available on the school website using the link -> Arranounbai School Medication Authority Form. If you have a Medication Authority Form in a different format, you can use that if it includes the same information.

If the medication is administered via Peg then you will also need to supply a supplementary page to the MAF. A template is available on the school website using the link -> Medication via Peg - Supplementary page

The documents must be unaltered after signed.
