I hereby give permission for the school Staff and externally funded service providers to discuss and work together at Farmborough Road Public School in supporting my child's development and an/or functional needs.

I give permission for Farmborough Road Public School to disclose information about my child with the provider to:

help support my child effectively; and
work with the Provider in ensuring that the Service delivered to my child aligns with their educational goals.

By signing this form, I understand that:

a) the School may collect information about my child from the Provider;

b) the School may also share information with the Provider;

c) the information collected may be discussed with the Provider, as is necessary, enabling to care for my child;

d) the information in a) and b) may include personal and health details about my child, such as the nature and implications of their medical condition;

e) the purpose of sharing this information is to assist the School in arranging and providing appropriate support for my child during school hours and school-related activities;

f) the School Principal may discuss this information with relevant Department staff to ensure appropriate care for my child;

g) the School will manage and store this information in line with the Department’s guidelines and legal obligations for information handling;

h) the School will not share this information with any third parties except as required by law or in accordance with legal obligations;

i) I can request access to the information disclosed by the School to the Provider;

j) I will notify the School if the Provider’s Service has been terminated;

k) I may withdraw my consent at any time, by contacting the School in writing. Withdrawing my consent will not affect any information already shared with the Provider prior to my withdrawal. Withdrawing my consent may however, affect how the Service is delivered by the Provider to my child at the School, or result in the Provider being unable to continue delivering the Service to my child at the School, if certain information is necessary for the delivery of the Service by the Provider.
