Permission Form

This form seeks your permission for {school_name} to disclose information about your child with the above provider (company) to:

help support your child effectively; and
work with the Provider in ensuring that the Service delivered to your child aligns with their educational goals.

By signing this form, you understand that:

a) the School may collect information about your child from the Provider;

b) the School may also share information with the Provider;

c) the information collected may be discussed with the Provider, as is necessary, enabling to care for your child;

d) the information in a) and b) may include personal and health details about your child, such as the nature and implications of their medical condition;

e) the purpose of sharing this information is to assist the School in arranging and providing appropriate support for your child during school hours and school-related activities;

f) the School Principal may discuss this information with relevant Department staff to ensure appropriate care for your child;

g) the School will manage and store this information in line with the Department’s guidelines and legal obligations for information handling;

h) the School will not share this information with any third parties except as required by law or in accordance with legal obligations;

i) you can request access to the information disclosed by the School to the Provider;

j) you will notify the School if the Provider’s Service has been terminated;

k) you may withdraw your consent at any time, by contacting the School in writing. Withdrawing your consent will not affect any information already shared with the Provider prior to your withdrawal. Withdrawing your consent may however, affect how the Service is delivered by the Provider to your child at the School, or result in the Provider being unable to continue delivering the Service to your child at the School, if certain information is necessary for the delivery of the Service by the Provider.
