This form can be used to request an External Service Provider to come to Arranounbai School for a ONE-TIME observation at Arranounbai School.

If you would like to request for a therapist to come to school for regular therapy sessions, please use the Parent/Carer Request for Regular Therapy with External Provider at school.

Observations will only be approved if they support the student accessing the school curriculum.

Once this request has been approved, we will contact the external provider to submit the required Department of Education compliance documentation. A time will only be scheduled once all necessary documentation has been completed and verified.

A minimum of two weeks notice is required for an observation.

You will receive notification once the form has been processed and the observation has been scheduled.
